Medical Leave

A student desiring to withdraw from classes due to medical reasons and who intends to apply later for reinstatement to the program will need to provide a letter from a health care provider documenting the need for medical leave. The letter should be addressed to the Associate Dean of Academic Programs and Student Affairs. The student will need to fill out a withdrawal form which can be obtained from the Dean’s Office. The student will be responsible for removing all personal items from their assigned locker, mail boxes, and study carrels within 10 days of the withdrawal. The student will be requested to communicate regularly with the Associate Dean of Academic Programs prior to reinstatement and follow the procedures of reinstatement in the Student/Faculty handbook.

Guidelines for students taking medical leave to consider upon petitioning for reinstatement.

  1. Obtain a letter from health care provider (therapist, licensed psychologist, psychiatrist, physician, disability services, etc.) stating:
    1. Diagnosis
    2. Prognosis
    3. Written verification that symptoms that led to medical leave have been alleviated and that if the student is medicated that they present no safety threat to themselves or others
    4. Recommendations for accommodations in the classroom, during examination, during clinical rotations
